Modernization/Clean-Up

• UFILES routines extracted from TRANSP
• ezcdf moved to an external library
• plasma_state and many of its dependencies, including xplasma, moved to an external library
• lsode and lsode_linpack replaced with a more modern ode solver
• Many portlib subroutines deleted and/or replaced with Fortran intrinsics
• Removed several unused variables related to PTSOLVER
